 Volunteer Clearance Instructions


All school-based volunteers must be cleared by the Human Resources Department for the 2021-2022 school year.  Please complete the new 21/22 volunteer/chaperone application online at this link:




Please be sure to use your legal name on the application, and to include your birthday and social security number when prompted so that HR can run the background check. If you have any problems with your login or password, please contact our district IT helpdesk at 480.484.HELP, or by opening an online work order through Let’s Talk (scroll down to “District Level Departments” and then choose “Communications”) at the following link:   https://www.susd.org/Page/4359


  1. Please email Hopi secretary, Danielle Leivian at dleivian@susd.org as soon as step #1 is completed. If you use a nickname or alias which is different from your legal name, please let us know so we can locate your application.
  2. A background check will be processed by HR.  This step will take an additional 5-10 business days to process, so please allow extra time for this step. There is no cost to you and, once you are cleared, the background check is good for (3) years and is transferable to other SUSD schools.
  3. Please print and sign the “Volunteer/Chaperone Covid Waiver” found here.  Hard copies are also available upon request.
  4. Please provide a copy of your vaccine card or proof of natural immunity*.  You can upload it directly to your volunteer application, or email it to Kailey Brown at kaileybrown@susd.org , or bring it with you to your HR appointment (step #6).  Please note, there must be two (2) weeks elapsed between the date of the last vaccine dose and the first date of volunteering on campus.  People who are freshly vaccinated must wait two weeks to volunteer.
  5. As soon as possible, please contact Kailey Brown in HR to schedule an appointment to pick up a badge at kaileybrown@susd.org or 480.484.6224.  Sorry, no drop-ins please.  Please bring along with you to the appointment: a picture ID, the signed “Volunteer/Chaperone Covid Waiver”, and a copy of your vaccine card or proof of natural immunity*. Kailey Brown is located at the SUSD District Ed Center, 7575 E. Main Street, in Old Town Scottsdale.  Thank you for being patient with Kailey as she is processing a high volume of volunteers for 29 schools.    

*Per the district, proof of natural immunity includes any (1) of the following:

  • Documentation of previous COVID-19 infection within 90 days (i.e., copy of the positive PCR test) OR
  • Antigen lab result or a physician’s diagnosis (include name and date of diagnosis) OR
  • COVID-unvaccinated individuals, or those with a COVID diagnosis of more than 90 days ago, MUST submit proof of a negative COVID test with sample collected no more 72 hours prior to volunteering on campus to Nurse Ali at akasulaitis@susd.org by 4pm the day prior; notice must include name, date and time of test.
